In approximately August 2010, the gaming website
Totaljerkface allegedly suffered a data breach that impacted 188k customer records. The breach included Email addresses, Usernames, IP addresses, Locations, User websites and Passwords stored as bcrypt ($2y$05) hashes. It was later found out that this breach was fabricated, and comes from another database.
